Heartworm is a classic survival horror with elements of a psychological thriller. A girl named Sam with cut legs leads a secluded lifestyle and does not want to leave her home. From the very beginning, the game hints that due to a difficult childhood, the young woman began to have mental problems. One day, she receives a videotape with strange contents. After watching the recording, the heroine suddenly wakes up in a completely different place. There, she begins to experience a sense of déjà vu.

Heartworm imitates "Resident Evil", "Silent Hill" and other seminal titles for the genre. The graphics in the project are stylized as console games of the late 20th century. The visual style intentionally uses low resolution and a small number of polygons. The indie project offers old-school gameplay.

Despite the third-person view, the camera here is fixed, and with different angles. The heroine, even within the same room, moves between screens.
